    ABOUT THE POSITION

    This position is responsible for following each assigned client’s individualized care plan, which includes assisting clients with daily living activities at the client’s home or care facility. You will be committed to ensuring your clients are satisfied with the service and quality of the care they receive.


    ESSENTIAL DUTIES AND RESPONSIBILITIES

    • Perform all duties per the client’s individualized care plan as directed by the home care supervisor or member of care management team.

    • Assist with all activities of daily living (ADL) as directed by the care plan. This includes, but is not limited to:

    o House services: housekeeping, laundry, dishwashing, etc.

    o Medication assistance (as delegated)

    o Personal care: bathing, showering, toileting, grooming, dressing, etc.

    o Meal preparation

    o Shopping and transportation

    o Repositioning/transfers/lifting

    o Standby assistance

    o Companionship

    • Communicate with care supervision team any changes in client’s condition, requests for scope of care outside of the individualized care plan, or requests from client for services outside of scope of license or credentials.

    • Provide emotional and social support as required by client preference or as directed by the individualized care plan. Actively engage clients in a manner that promotes their independence and maintains their dignity to perform necessary tasks and activities of daily living (ADL).

    • Accurately maintain client’s records in a timely manner and in accordance with company policy. This includes but is not limited to:

    o Documentation of medication distribution

    o Leisure activities

    o Reportable incidents

    o Client observations

    o Changes to health or emotional condition of the client

    • Immediately report all changes in condition to the home care supervisor and/or care management team.

    • Ensure that the environment remains safe, clean, and orderly for the client. Perform general housekeeping duties and maintain cleaning schedules for client’s laundry, bedrooms, dining area, living space, bathrooms, kitchen, and other areas as directed.

    • Safely transport clients to and from appointments, errands, activities, etc. as needed and/or directed by the plan of care.

    • Assist clients with ambulation and mobility by safely assisting with mobility and transfers of clients.

    • Adhere to proper safety protocols and company procedures in emergency situations.

    • Assist client with physical therapy activities as directed by the plan of care.

    • Respond promptly and positively to client requests for assistance.

    • Consistently provide excellent hands-on care and bedside manner with patients.

    • Regularly and constantly report to all scheduled shifts. Accurately record all time worked for scheduled shifts via Telephony or WellSky Personal Care phone applications.

    • Conduct self professionally and knowledgeably while representing Fedelta Home Care to clients.

    • Maintain strict confidentiality with sensitive information and proprietary company information. Adhere to HIPAA privacy practices and confidentiality as required by regulation.

    • Maintain a high level of professionalism in conduct and communication with team members, clients, and families while engaged in work responsibilities for Fedelta Home Care.

    POSITION REQUIREMENTS

    This position requires strong time management skills, interpersonal skills, patience, listening skills and knowledge of generally accepted care standards. This position requires the ability to communicate effectively in English, both verbally and written communication skills; through various channels including in-person, phone, email, and text messages. This position requires demonstrated experience and tact in communicating with diverse populations. This includes the ability to recognize and understand the differences, needs, experiences, values, lifestyles, and ages of the populations served. Must have the ability to maintain a high level of professionalism and customer service standards in stressful situations. This position requires regular local travel and reliable transportation to assigned client locations.

    SUPERVISORY RESPONSIBILITIES:

    None. Escalated matters of significance are referred to Home Care Supervisor or Manager.

    CERTIFICATIONS AND/OR REGISTRATIONS:

    RN Delegation certification is highly desired. A valid Washington State Driver’s License or Oregon State Driver’s License, satisfactory driver abstracts, and proof of auto insurance are required for individuals that drive while engaged in duties for Fedelta Home Care. Must have the ability to successfully complete



    EDUCATION AND/OR EXPERIENCE:

    Requires a minimum of six months industry related experience.

    • For Washington State employees: Certified Nursing Assistant (C-NA), Home Care Aide (HCA), Home Health Aide (HHA) license is required and must remain in good standing during employment.

    $20.60-$23.00 per hour
